Discharge from hospital does not mean recovery is complete.
For many patients, the most challenging phase begins at home, such as managing medication schedules, mobility limitations, wound care, rehabilitation exercises, monitoring symptoms, and coordinating with family members or informal caregivers.
Instructions are often delivered in brief discharge summaries or paper sheets. Once at home, uncertainty increases. Caregivers may feel overwhelmed. Patients may struggle with confidence, memory, or physical mobility.
AI and XR technologies open the possibility of more contextual, guided, and adaptive support in home environments (e.g., helping patients visualise care routines, rehearse procedures, and receive structured guidance from pre-trained agents and healthcare providers when needing human-in-the-loop support).
This track invites teams to design systems that support recovery at home in realistic, bounded, and responsible ways.
